Limitation of Current Equipment:
__________ is currently using a commode chair that puts him in an unsafe situation where he is tilted too far forward because of the tilt mechanism, or he is tilted fully back and unable to do anything to contribute to his shower and becomes agitated with his positioning in tilt. In attempting to lessen the tilt but maintain some support, the mechanism releases and he is thrown forward in the chair. This is extremely unsafe for him and also for his caregiverr as they attempt to support him.
Equipment Recommended: Broda CS 385 Commode Shower Chair
It has been recommended to use the Broda CS 385 Chair for ________. It is the most supportive and substantial and the hydraulics system is the most suitable for him. It can be adjusted to suit __________ via tilt, but also be using the back rest recline as needed. The Broda gives him more support so he is willing to participate in his personal care.
The Broda Chair also has an all in one foot support, which is suited to him as he likes to cross his legs which places a lot of weight on his current single footplate/legrest but is no problem on the full support of the Broda.
Expected Measurable Outcomes:
____________ will be able to participate in his personal care each morning. He will be safe at all times whilst in the chair. ___________’s caregiver will be safe at all times whilst providing his care.
Findings:
The Broda Commode Shower chair meets _________’s needs. He indicates he feels safe and has been attempting some of his personal care whilst in the chair- ex. hosing himself, reaching up and washing his hair, lifting his leg right up so caregivers could dry under it. All staff report being pleased with the commode and find it much easier to assist him.
