Please measure the following sitting in an upright position with a measuring tape:

1. Seat Depth
   1. The distance from back of the buttocks to popliteal (underside of the knee).
2. Lower Leg Length
   1. Feet fiat on the fioor & knees bent at 90 degrees, measure from the popliteal fold (underside of the knee) to the fioor.
3. Seat to Top of Head
   1. The distance from the top of the head to the base of the spine.
4. Seat Width
   1. While sitting on a firm surface, measure the width of the buttocks.
5. Height
   1. Measure height from heal to head.
6. Weight
   1. Record weight at night (preferable).
