How to assess if someone is at fall risk

Questions to Ask:

  1. Does the person tend to slide?
  2. Do they try to stand up when they are unable to do so?
  3. Do they currently use a restraint such as a lap belt?
  4. Do they get agitated when they can’t move around?
  5. Do they have a history of falls, and under what circumstances did those falls occur?
  6. Do they bang their chair into furniture or other residents?
  7. Are they in pain from their current chair causing them to try to get away from this pain?
